Below are different opportunities for students to get a sports physical this year. These clinics are FREE and open to all students needing a sports physical for this school year.

These clinics are provided by The Iowa Clinic. The attached form will be provided onsite, but for faster processing, we recommend completing the first 3 pages prior to arriving at the building.
